Direct Support Professional
Direct Support Professionals work with individuals with mental or developmental disabilities by creating behavior plans, teaching self-care skills, cooking meals and doing laundry. Additional duties may include:
- Assisting clients with daily personal tasks, including bathing and dressing
- Completing housekeeping tasks such as vacuuming, washing dishes and tidying
- Helping plan client appointments and organize a schedule
- Organizing with transportation arrangements to appointments
- Shopping for groceries and preparing meals that meet specific client dietary needs
- Encouraging client engagement in social networks and communities
Case Manager
- Physical Requirements: Must be able to lift up to 30 pounds. COVID-19 vaccination required.
- Access to reliable transportation and the ability to occasionally work from home.
- Case managers help connect individuals to resources and services in their own communities
- Case managers work with adult consumers to identify their interests, strengths and vision for their desired lives, and to develop plans to help them achieve their goals
- Effectively coordinate service requirements and referral of clients to appropriate community resources.
- Advocate and collaborate with other service providers in the provision of services on behalf of clients.
- Develop and maintain a supportive counseling relationship with clients and affected others, drawing upon knowledge and skills in mental health, substance misuse and addictive behavior, grief and bereavement, trauma recovery.
- Part-time position
